CENTER FOR HEALTH SERVICES

 

The Center for Health Services (CHS) provides effective school health program to its clientele. The CHS aims to protect and promote the health of its clientele to acquire knowledge and skills for self-care, foster growth and reinforce positive health attitudes. The Center’s services include health education, communicable disease control, environmental safety and sanitation, and provision of first aid.

 

The Center for Health Services (CHS) contributes to the physical, mental, and social development of pupils/students and personnel through health promotion, maintenance of health and good nutrition, and disease prevention geared towards the attainment of the institutional vision and mission of developing Ignacian Marian leaders of faith, excellence, and service.

  • Medical and Dental School Records
  • Annual Physical Examination of Personnel
  • Daily Consultations and Treatment
  • Teleconsultation Services - from 8:00 a.m. to 4:00 p.m. for pupils/students and personnel with COVID-19 symptoms / confirmed COVID-19 patients / symptomatic / with symptom/s close contact to a COVID-19 patient
  • Referral of Emergency Cases to Hospitals
  • Health Teachings / Bulletins
  • Sanitation Inspections (Toilet and Food Services / Canteen Inspections)
  • Water Microbiological Examination of Food Services / Canteen and Drinking Fountains
  • Implementation of National Health Programs of the Department of Health through the Local Health Department (Quezon City Health Department and Roxas Health Center)
  • Implementation of School Health Programs through the Philippine Private School Health Officers’ Association, Inc.
  • Extension of Services to Parents / Official Guardians of Pupils/Students and Immediate Family of Personnel and Visitors
  • Breastfeeding Area for Lactating Mothers

 

GUIDELINES ON PATIENT ADMISSION AND CONFINEMENT

Before Treatment

1.    Only the patient should enter the Center for Health Services.
2.    The patient / School Nurse should log in the patient’s name on the Daily Consultation and Treatment Logbook.

 

During Treatment

1.    Patient’s condition will be assessed.

a.    Patient is allowed to stay if he/she is under observation for:

•    allergic reactions
•    chest pain
•    communicable / infectious disease
•    diarrhea
•    difficulty of breathing
•    dizziness
•    fever (37.8 ºC or higher)
•    increased body temperature (Temperature of 37.3 - 37.7 ºC)
•    severe abdominal pain
•    severe headache
•    vomiting

b.    Patient will be allowed to go home if:

•    the abovementioned conditions are not relieved.
•    COVID-19 related symptoms are present.
•    diagnosed with communicable disease such as:
          - chicken pox
          - hand-foot-mouth disease
          - measles
          - mumps
          - sore eyes
          - suspected COVID-19 symptom/s and
          - other related diseases
•    he/she is in need of immediate dental treatment.
•    he/she must be referred to a medical specialist or to a physician if the school physician is not around.
•    he/she suffered injuries that need immediate intervention such as:
          - fracture
          - laceration
          - deep punctured wound and
          - other related injuries

2.    Patient will be given medicines only if:

- medicines are prescribed by an attending physician and prescription is presented.
- the patient has undergone consultation with the school physician.
- a consent is given by a parent or official guardian.
- medical history forms are updated with medicines specified in case of fever, toothache, allergies, and asthma.

 

Note: Taking of medicines belonging / prescribed to other pupils/students is strictly prohibited.


3.    The patient will be given recommendation slip/s as needed and if seen by the school physician. Prescription/s will be given if necessary.
4.    For emergency cases, the patient will be brought to the nearest hospital. Parents / Official guardians are immediately informed about the case.
5.    Only one (1) patient is allowed on each bed.

 

After Treatment

A.    Going back to the classroom

After having been relieved from his/her complaint, the patient will be advised to go back to the classroom. He/She should log-out, write the complaint, and treatment done in the Daily Consultation and Treatment Logbook.
Preschool pupils, Junior High School and Senior High School students will be issued a CHS Slip signed by a Health Services Personnel to be submitted to the Class Adviser/ Subject Teacher.

B.    Going Home

1. The CHS Personnel will inform the Class Adviser through the Basic Education Department Office if the patient will be sent home. Only the school personnel and Emergency Preparedness and Crisis Management Committee (EPCMC) members shall assist the patient with his/her personal belongings.
2. The CHS Personnel will inform the parents/ official guardian if the patient will be sent home. Otherwise, he/she will NOT be allowed to go home.
3. Before going home, the patient should log-out, write the complaint and treatment done and should secure an Exit Pass to be signed by any of the CHS Personnel. The Exit Pass will be submitted to the Security Guard on duty at the gate before a patient is allowed to go home.
4. Fetcher should log-in and out in the Fetcher’s Logbook.

 

Note: Any pupil/student exhibiting COVID-19 symptoms needs to be fetched immediately or at the earliest time possible after the call to the parent official guardian is made.


Medical Clearance:

Those who had been absent for two (2) or more days due to prolonged sickness / any health concern / hospitalization shall seek first a medical clearance from the Center for Health Services provided that they present a medical certificate before reporting back to school. Those who failed to comply will be referred to their respective Discipline Coordinator/s as the medical clearance is a requirement to screen the pupil/student before allowing to return to school.

Pupils/Students who have been absent due to infectious / communicable diseases such as sore eyes, mumps, chicken pox, measles, hand-food-mouth disease, and other related diseases are required to present a medical certificate from their family / attending physician and seek medical clearance first from the Center for Health Services before entering their classrooms. Such will determine if they are fit to go back to school or still need to be isolated / referred to family / attending physician.

 

Note: The Center for Health Services has the authority to send a pupil/student home to rest / isolate for a prescribed observation period if and when the Health Services personnel has  assessed that the pupil/student is not yet fit to report. Absences incurred during the prescribed observation period will be given consideration by the concerned school authorities.

 

Pupil/Student will be issued a CHS Slip signed by the Health Services personnel if he/she is given clearance to report to school. CHS Slip shall be submitted to the Class Adviser / Subject Teacher.
 

Other Relevant Information:

Pupils/Students with recent health problems must report these conditions to the Basic Education Principal and the Center for Health Services through correspondence at the beginning of classes or when a problem arises. These health concerns could include, but are not limited to, vision or hearing deficiencies, diabetes, seizure disorder, asthma, life-threatening allergies, chronic illness, recent surgery, contagious illnesses, symptoms of contagious illnesses, medication or anything that might limit your pupil’s/student’s participation at school.

 

Pupils/Students are only allowed to stay and rest in the Center for Health Services if they are sick and may stay there until their condition is stable and vital signs are normal. Pupils/Students are NOT allowed to standby / loiter inside the Center for Health Services.

 

Pupils/Students are advised to have their breakfast before coming to school to prevent dizziness, stomachache or headache.

 

Patients shall keep noise to a minimum while waiting inside the Center for Health Services.
 

 

GUIDELINES ON TELECONSULTATION SERVICES FOR PUPILS/STUDENTS AND SCHOOL PERSONNEL

The Center for Health Services (CHS) is offering medical consultation through telephone call and Microsoft Teams with the CHS School Physician and School Nurses for Non-emergency and Non-life Threatening Health Concerns with the aim of providing them support. A communication device like landline telephone / mobile phone / laptop / desktop computer is needed to avail of the Teleconsultation Services.


I.    GENERAL CONDITIONS:

1.  Assessment, Advice, and Recommendations are based on all information or data shared / provided by the pupil/student, parent / official guardian / Class Adviser, College students, College student’s Program Head / Associate Dean and school personnel during Teleconsultation.

2.  Teleconsultation Services of CHS is NOT recommended for those pupils/students who are required to have examination or diagnostic procedure that can only be performed in-person by a physician.

 

3.    WHO CAN AVAIL OF THE TELECONSULTATION SERVICES?

3.1    Pupils/Students with updated Medical History Forms, a duly accomplished online form through Microsoft Forms by the parent/ guardian

         (for Basic Education pupils/students) and College students

3.2    School Personnel

3.3    Pupils/Students and School Personnel who need medical advice and experiencing mild symptoms such as:

    colds
    cough
    headache
    joint pain
    muscle pain
    skin allergy
    skin irritation / itchiness
    tolerable abdominal / stomach pain
    constipation
    watery stool

      

Note: Basic Education pupils/students will need the guidance / supervision of their parent/ guardian in availing of the teleconsultation services of the Center.

 

3.4    Pupils/Students and School Personnel who are under isolation and are suspected/ confirmed cases of COVID-19

3.5   Pupils/Students and School Personnel with recent health problems or had been absent for two (2) or more days due to prolonged sickness / any health concern that might limit a pupils’/students’ / personnel’s participation during Face-to-Face classes and job functions.


4.    WHO ARE ENCOURAGED TO SEE A DOCTOR IN PERSON?

4.1    Pupils/Students and School Personnel who are having:

•    blurred vision
•    chest pain
•    coughing with blood
•    defecating with blood
•    difficulty of breathing
•    head injuries caused by trauma
•    heart palpitation
•    loss of consciousness
•    weakness and numbness of extremities
•    others, which will require the physician to conduct/ perform physical examination and to request for a diagnostic test

 

II.    REPORTING AND RECORDING

1.  The School Nurse shall record the Teleconsultation done.

2.  The School Nurse shall inform the Class Adviser, Associate Dean/ Program Head, and Department/ Unit Head/ Coordinator of the patient on the medical advice and recommendation/s given as needed / necessary.
